Community Care Officer – Myrtleford
Fixed term (17 months- Minimum .6 EFT)
Alpine Health requires a Community Care Officer for our expanding alpine@home Community Care Service to work across the Alpine and Indigo shire communities. The Community Care Officer role provides an opportunity to undertake Case Management and Care Coordination for our Home Care Package program. Working as part of a dynamic team you will be responsible for providing comprehensive case management for our Home Care Package clients.
If you have a passion for Community Care this role provides a challenging, varied and rewarding opportunity. A proven ability for quality and responsive case management and care coordination services to older people is a strong requirement of the role.
Applicants are expected to have a current police certificate within a 12 month currency, current first aid, current driver’s license and an immunisation status consistent with Alpine Health’s Staff Immunisation Procedure.
