Home and Community Care Program for Younger People (HACC PYP) 

The Home and Community Care Program for Younger People (HACC PYP) provides support for younger people with disabilities whose capacity for independent living is at risk.

Eligibility

Services are targeted to younger people with moderate, severe or profound disabilities and their unpaid carers. You may be eligible for support if you;

  • have a disability or care for person with a disability
  • you been assessed as not eligible for NDIS
  • are aged under 65
  • need assistance with the activities of daily living

Services

The main types of HACC PYP Services are:

  • domestic assistance
  • personal care
  • nursing
  • food services (meals on wheels or centre based meals)
  • planned activity group
  • property maintenance
  • respite services
  • community transport services

Note: Services may vary from Shire to Shire. Please discuss with the HACC PYP Assessment Officer.
