Community Home Support Program (CHSP)

home care services to help you at home

The Commonwealth Home Support Programme (CHSP) provides entry-level home care support services to assist older people aged 65 years and over* to live to safely and independently in their homes. 

Our services cover the Alpine and Indigo valley's including Bright, Myrtleford, Mount Beauty, Beechworth, Yackandandah, Chiltern, Rutherglen and Tangambalanga and NE Victoria. Services in the Alpine Shire are delivered by alpine@home. Services in the Indigo Shire are delivered by indigo@home in partnership with Beechworth Health Services and Yackandandah Health.

Eligibility

To be eligible for these funded services you must meet some needs and age requirements:

  • how much assistance you need with some everyday tasks
  • that you are 65 years or older (*50 years or older for Aboriginal or Torres Strait Islander people)

My Aged Care is the starting point for all funded aged care. You can check your eligibility by contacting My Aged Care;

How to access services

Register with My Aged Care

Call My Aged Care (MAC) on 1800 200 422 to register for an assessment or apply online at My Aged Care.

Have your medicare card with you and allow enough time to answer some questions about eligibility, your details, and next steps for an assessment.

Assessment

The My Aged Care Assessor will contact you to arrange an in-home assessment or telephone assessment. They will ask you to nominate a service provider.

Choose Alpine Health as your service provider

When you choose Alpine Health as your service provider, My Aged Care will send us your assessment details. We will develop your support plan according to your assessed needs.

Support

The Alpine Health Intake Team will get in contact to review your support plan and confirm your services so you can get back to doing the things you love.

Costs

Services are subsidised by the Commonwealth Home Support Program (CHSP) depending on your needs and circumstances.  Alpine Health will work with you to determine the level of your contribution towards your services.

Note: Low Level Fees apply to all Health Care Card holders and people receiving a full or part pension.

What if my needs change?

You will have an annual review of your support plan to determine if your needs changes.

If you can’t wait for your annual review, please call us first on 03 5755 0142 to review your support plan to see if we can add more services.

If your needs cannot be met within the Commonwealth Home Support Program, you may need a reassessment by My Aged Care to explore other options for more complex care needs like Support at Home.
